Taylored Healthcare was founded by Leslie Taylor, DNP, APRN, FNP-BC, a board-certified Family Nurse Practitioner with over eight years of provider experience and a passion for delivering high-quality, relationship-focused care. Leslie earned her Doctor of Nursing Practice degree from Murray State University and has advanced certifications in nursing education and healthcare policy. Her vision for Taylored Healthcare is to create a modern healthcare experience where patients feel heard, valued, and supported while receiving convenient, personalized virtual care that truly fits their lifestyle.

This plan is ideal for patients managing chronic conditions, needing regular follow-up, or wanting greater flexibility and access to personalized healthcare services. This membership offers enhanced access, continuity, and convenience while fostering a strong provider-patient relationship through affordable and transparent virtual primary care.
Couple & Family Memberships
The Couples Membership is designed for two adults in the same household seeking convenient, affordable, and personalized virtual primary care. This plan offers comprehensive telehealth access for both members while promoting continuity of care and simplified healthcare management for couples. The Family Membership is designed for two adults and up to two dependents, providing convenient and comprehensive virtual primary care for the entire household. This plan offers affordable healthcare access with personalized support tailored to the needs of growing families. Additional dependents may be added for $35/month each. This membership is designed to simplify healthcare for busy families by offering accessible, transparent, and relationship-focused virtual care from the comfort of home.
